Planning Your Visit
- Request care: call 281-589-8877 or request an appointment online. Our team will contact you to confirm a time. New patients can complete the New Patient Form before the first visit.
- What to bring: your physical therapy referral or prescription if you have one, your insurance card and your payment information. For Workers’ Compensation, bring your claim number and your case manager’s contact information; for auto insurance or an attorney lien, bring that information too.
- What to wear: loose, comfortable clothing so your therapist can see and move the area being treated.
- Your first visit: your therapist reviews your history, symptoms and goals, examines movement, strength and the affected area, and builds a treatment plan with you. Sessions typically last 60 to 90 minutes.
- Insurance and payment: we accept many insurance plans and recommend verifying your benefits before your first visit. Copays, coinsurance or deductibles are due at check-in. We also accept cash, checks, major credit cards and CareCredit, and offer self-pay packages.
- Referrals in Texas: you can be evaluated without a referral, but continued treatment may require one. See our FAQ or contact us with questions.
